Reablement Care
Nexxus Care is commissioned to deliver a reablement programme across East and South Staffordshire. The primary aim of this short-term, intensive support programme is to help individuals regain independence, reducing or eliminating the need for long-term care.
We promote a “do with” rather than “do for” approach, encouraging and supporting individuals to rebuild their confidence and daily living skills.
Our service includes a dedicated team of:
Occupational Therapists
Senior Support Team Leaders
Support Workers
What to Expect
Where it is safe and appropriate to do so, Nexxus Care accepts referrals from social care teams or hospitals.
A Senior Team Leader will:
Arrange to meet you at home following discharge, or shortly afterwards
Complete an initial assessment
Develop a personalised care plan tailored to your needs
Your Personalised Care Plan
Your care plan will:
Outline any support needs our team should be aware of
Set clear, achievable goals based on what you want to accomplish during the programme
How We Support You
Our team will work closely with you to achieve your goals, with guidance from an Occupational Therapist. Support may include:
Rebuilding daily living skills
Improving mobility and confidence
Introducing assistive technology where appropriate
Monitoring Progress
Your progress will be regularly reviewed throughout the programme. If you, the team, and the Occupational Therapist agree that your goals have been achieved, the reablement programme can be successfully completed—typically within 28 days.
If Further Support Is Needed
If ongoing needs are identified:
The team will notify the local authority
Arrangements will be made for longer-term support, such as domiciliary care within the community.
