You will be supporting service users to regain their independence once being discharged from hospital whilst also preventing prolonged hospital admissions.

? We are looking for reablement care assistants who are flexible to cover local locations across Staffordshire and East Staffordshire. Therefore, you will be required to have a full driving licence with access to your own vehicle. ?

Your duties will include personal care, supporting with medication, following a clients personalised care plan, and working closely with our senior team and occupational therapists to achieve individual goals. Experience is preferred but not essential as full training will be provided.
